Voter Vault Forms-Click Here  for the Voter Vault Application(.pdf file).  Print, fill out the form, sign and mail to Executive Director Geri Davis at PO Box 723, McHenry, IL 60051.    Geri will forward your request, signed by the County Chairman, to Voter Vault.  You will receive your User ID and Password via email.  It usually takes about 10 days to process a Voter Vault Application.
